  1. Dear Sir/Madam,

    I joined the National textile corporation in April, 2013 and was posted at Finlay Mills,Achalpur which comes under the Akola Region for PF purposes and accordingly i was alloted the UAN No. as 1001 590 88631 with name wrongly printed as GODABA SHRINIVAS PATNAIK and the same was brought to notice of the officials immediately.

    Later i was transferred to our western regional office at Mumbai and i have provided the previous UAN No.1001 590 88631 but to my surprise, my PF got deposited under the new UAN No.100932827769 under the name wrongly printed as G S PATNAIK and the same was brought to notice of the officials immediately.

    When i checked, i found that the PF balances are appearing under both the UAN No’s and my correct name is not updated in either of the UAN No’s.

    I was in constant touch with both the concerned offices of my organisation and nothing was materialised, i tried online.

    I tried to update my KYC with PAN card details as well as Aadhar card details but in both the UAN , the details didnot update.
    The name as per my PAN card/Aadhar card is G SRINIVAS PATNAIK.

    Hence, i requested the employee feedback portal an email dated 11th Oct,2017 employeefeedback@epfindia.gov.in to integrate the balances under both UAN’s into any single UAN and also update my name in the records as per my PAN CARD or Aadhar card.

    For kind reference,I had enclosed the scan copy of
    1) PAN card 2) Aadhar Card 3) UAN id card-previous 4) UAN id card-current.

    As advised as per trialing mail received from employee feedback,i have sent the filled in form to my previous place of posting i.e finlay mills,Achalpur and they said that the request has already submitted to PF office, akola office for updating my name in your records as per AADHAR CARD but till date the same is not updated in your records.

    I have been transferred on 11th October,2016 from finlay mills,achalpur but there is an inordinate delay in processing my request since last 1 year, 7 months.

    So, i request all the concerned to sort out the issue by updating the name in the PF records as per the details as per my aadhar card and also do the needful so that the balance is transferred to any one EPF account under One UID.
